There are more than 8 million visits to emergency departments for chest pain or other symptoms consistent with myocardial ischemia annually in the U.S. However, only a minority of these patients have a life-threatening condition. Similarly many patients with palpitations present to the Emergency Room and are admitted unnecessarily. 

PCA’s AUH program is designed to expedite outpatient care and management for chest pain as well as dysrhythmia and other non-hospital requiring conditions with confirmed immediate (within 24 hours) outpatient reassessment and follow-up with a PCA cardiologist. 

We strive to do the right things for the right reasons and do them right the first time! Limiting readmissions is part of an ongoing strategy to improve care coordination and utilize resources judiciously at PCA. PCA’s AUH Program has the ability to decrease patient 30-day readmissions and return-to-ER visits through the integration of discharge planning, post-discharge follow-up and care management interventions.
